Evidence-Based Rationale
Psychedelic therapies are safe in controlled settings with proper screening (minimal adverse events in trials). Risks (e.g., transient anxiety, blood pressure elevation) are mitigated through exclusion of high-risk individuals (psychosis, bipolar I, uncontrolled cardiovascular disease) and thorough preparation (set intentions, build rapport, regulate nervous system).
Absolute Exclusions (Contraindications)
Current or history of primary psychotic disorder, schizophrenia, schizoaffective disorder.
Bipolar I disorder (risk of mania activation).
Active severe suicidality or serious risk to others (C-SSRS assessment).
Uncontrolled hypertension or significant cardiovascular disease (arrhythmia, recent MI/stroke, QTc prolongation >450 ms males / >460 ms females).
Pregnancy, breastfeeding, or trying to conceive.
Severe liver impairment or acute substance use disorder (other than mild-moderate cannabis/alcohol).
Known allergies or severe adverse reactions to relevant medicines (e.g., ketamine/esketamine).
